Komarisa was in the bottle, most likely due to an A-sshole-non stuffing her in it since the bottle was still upright. Marisa knocks over the bottle so the Ko can escape, but it can't get out. Patchouli, the mother, suggests to Marisa to tip the bottle so the Komarisa will slide out, while her little sister Patchouli encourages Komarisa. Marisa then tries lifting the bottle only for it to slip and roll away, hurting Komarisa. Komarisa gets upset, and says she "doesn't know a father who hurts her" and would rather have her mother rescue her, which of course shocks Marisa. Ko then rolls towards the Patchoulis... only for momentum to keep her from stopping and squashing her little sister to death, which in turns kills Komarisa either from the impact or shock or both. Marisa gets pissed and stomps on the bottle telling it to die, only to slip off it as it slams into mother Patchouli and kills her. Marisa herself just seems knocked out(the Reimu and Alice are like the little dizzy birdies,) but now her entire family is dead because of a bottle.
