From the way it's drawn, it took me a looooong time to figure out, but I _think_ the top right Reimu is having the skin of her footsies scrubbed away by a stiff-bristled (tooth?)brush. (For the longest time, I saw the handle as the ground and the bristles as motion lines of hopping.)
