It is surprising if you have a mistaken image of the creature doing so, thinking it's "too pure and innocent" like to do such things. If I recall correctly that was part of the reason why Piko didn't like his classmates lusting over bodied yu's, aside of the whole "darned horny toads", this whole thing proved her wrong shockingly.
It's like you thinking your neighbour is a pure and refined lady above any form of depravity, only to find her in a very raunchy and hardcore position with some unknown gentleman.
But honestly, seeing a yukkuri refresh with a doll is about as shocking as a dog humping a table leg. Annoying, maybe, but certainly not surprising.

It's not inherently unusual yukkuri behavior, but at the same time I don't think it's specifically about her perception of the sexualization of yukkuri.
She was stunned to find out the things peed and pooped after eating and drinking. Of course she's going to be speechless to find it refreshing.
