I wonder what caused the little Reimu to contract diarrhea, or whatever it is for yukkuri.
Maybe Maricha only did hunt for sweet stuffs instead of good foods. Since yukkuri parents usually know it's better to eat certain foods instead of sweet sweets only, they feed their children with bugs and grasses before even giving them any sort of candies.

I would say it is because of the food big sis marisa fed her. Since she's inexperienced in gathering food, she would either bring her little sis non-nutritious food or something that outright make her ill.

Just my speculation
We have like 4 reasons why Wasa experiences diarrhea, and this is still increasing, here are some of them (chronological order)
1/ Mister cosmicapprentice said that because Maricha fed her sister "sweet stuffs instead of good foods", which sounds very unlikely to me, but okay.
2/ Mister Hitosura believes it's "anti-yukkuritis. From the death of their parents", very convincing.
3/ I think that it's because she "consumed way too much water", as Maricha relentlessly gave water to her sister, and usually, drinking water is one of the ways to lessen the hunger when one cannot eat.
4/ Mister Ieneko-Sasha speculated that Maricha brought home "non-nutritious food or something that outright make her ill", which is very plausible.
All of them are credible. Unfortunately, without any context whatsoever, we cannot find a real answer.
5. 9th wrote it that way because Reimyu has to die, Maricha needs to have a reason to try to help her sister and fail. To paraphrase one bad writer: "When a yukkuri wants to take it easy, all the universe conspires in making it suffer instead."
The death of their mother is exhaustion since her corpse is clean. Anit-yukkuritis causes vomit and shit vigorously.
Yes, but we are talking about Wasa, right? And no, shitting and vomit are one of the symptoms of anti-yukkuritis, but the vice-versa is not true. Lots of pool depicted that the yukkuri who suffered from that symptom do not shitting or vomitting, but just shouting something meaningless (like "easy easy easy").
As far as I know, anti-yukkuritis causes quick death. Wasa probably gets rotten food.
p/s: how anti-yukkuritis works is mystery for me. In Mohawk sensei story (Takumi), when touching or grabing yukkuri core, they have the same symptom - clap their hands and move their tongue. So I assume that spray attacks yukkuri's core.

Stuff.
Although I get what you're trying to say, Mister Yuno, I really don't think this is a good example of that phrase. Right from the start, we can clearly how this gonna end, I mean, a gutter trash family, there's almost no good outcome happen to a family of stray. So really, it doesn't surprising to see the family take it easy forever, unlike True Happiness, or that story when a Marisa and Reimu tried to move their house to a new place, but somehow it turned wrong due to the cap Marisa gave Reimu earlier.
