Tbh, I cringed (really hard) at the fourth one, pretty sure that if this one is an "official" method of giving birth of the yukkuri (since I have never seen anyone beside y drawn this), the number of birthkill and birth-death in yukkuri's world will drop drastically.
I feel the one big problem with backbirth is that they are coming out facing the wrong way. In the real world, when this happens they usually have to perform a C-Section, here what will most likely happen is the accessories will get stuck on the way out.

So this method has a much higher chance of accessory loss, which in turn raises the chances of death through misidentification. Interesting.
Nah, there is no different between "feet facing out" or "head facing out" in yukkuri's world. There is some pools that mention about this (can't find it right now, unfortunately), also, human's logic cannot applied to magical manju anyway. I think that this is like stalk birth, the accessories stick with yukkuri, unless something comes up.

I do believe there is something keeping their hats on, however I believe there is a big difference between the two. In stalk pregnancy, nothing really obstructs the offspring from coming out, it's just a simply nip from the bud. But with this back pregnancy, this is clearly not the case. The offspring are struggling violently to get out, causing a degree of pain upon the parent. With such an uneasy experience, it'd be undoubtable that hats might get stuck inside when the offspring finally breaks free.
